Doctor Herzenstube is the physician Madame Hohlakov has called in after Lise's reported improvement. He shrugs and says he is amazed, unable to explain why her fevers have ceased and her legs seem stronger. At this first appearance he stands for sober medical uncertainty beside Madame Hohlakov's excitement over a possible miracle.

Madame Hohlakov mocks him as the doctor who always comes, examines Lise, and says he can make nothing of it.
V-I. The Engagement
He is summoned for Katerina Ivanovna after her hysterics and delirium, and Madame Hohlakov jokes that he is so alarmed he almost needs a doctor himself.

Herzenstube is attending Ilusha as the boy's illness worsens, placing the town doctor inside the Snegiryovs' desperate household.

At the trial, he appears as an esteemed old doctor whose German turns of speech, stubborn opinions, kindness to the poor, and memory of Dmitri's neglected childhood briefly soften the courtroom.

After the trial, Herzenstube and Varvinsky attend Ivan while Katerina Ivanovna nurses him through fever.
